Multidisciplinary Collaboration Leverages Expertise: Critically Ill Colon Cancer Patient Receives Timely Life-Saving Treatment

Recently, a male patient was admitted to the Minimally Invasive Surgery Department of our hospital via emergency, presenting with abdominal pain, fever (high fever of 39°C to 40°C), left thigh pain, and poor appetite with no desire to eat.

At a friend's recommendation, Mr. Guo came to our hospital seeking help, holding a "try and see" attitude. The patient had experienced intermittent bloody stools for six years and left lower abdominal pain for five months, having sought treatment at various major hospitals without success. Three days prior to admission, his left lower abdominal pain intensified, accompanied by chills and high fever, with a maximum temperature reaching 39°C. He also had no anal passage of gas or stool, prompting him to seek further treatment at our hospital.

After admission, CT, MRI, and ultrasound examinations revealed a tumor in the descending colon complicated by perforation. The tumor had invaded the retroperitoneum and the lateral abdominal wall, forming a retroperitoneal abscess. The patient was diagnosed with severe infection complicated by intestinal obstruction, necessitating emergency surgery. Given the urgency, Dr. Teng Tingpeng's team convened a multidisciplinary consultation involving the hospital's Urology, Gastrointestinal Surgery, Orthopedics and Soft Tissue Surgery, Anesthesiology, Integrated Traditional Chinese and Western Medicine, and Radiology departments. Based on the patient's complex condition, they made personalized adjustments to the surgical plan. The patient's critical condition, including severe infection and intestinal obstruction, mandated emergency surgery to drain the abscess, remove the tumor, and perform a colostomy. Concurrently, factors like severe malnutrition significantly increased the surgical risk. After comprehensive multidisciplinary assessment, a detailed surgical plan was formulated.

With the concerted efforts of Deputy Chief Physician Yang Rong from Anesthesiology, Deputy Chief Physician Tang Juntian from Urology, and Deputy Chief Physician Yang Qianzi from Orthopedics and Rehabilitation, the complex procedure was successfully completed in just four hours. The surgery included: Cystoscopic ureteral stent implantation + Colon tumor resection with intestinal anastomosis + Ileostomy + Abscess drainage + Resection of the lateral abdominal wall soft tissue lesion invaded by the tumor!

Postoperatively, the patient was transferred to the ICU. Under the meticulous care of the Critical Care Medicine Department, his condition stabilized, and he was transferred to a general ward on the second day after surgery.
