Does a Positive Hepatitis B Serologic Panel in a Newborn Mean Hepatitis B Virus Infection?

A newborn refers to an infant from birth to under 28 days old. Many expectant mothers, especially those with hepatitis B, fear their child might be infected with the virus. Therefore, they are eager to have their newborn's hepatitis B serologic panel checked to see if the baby is infected. However, the results can be surprising because at this stage, regardless of whether the mother has hepatitis B or not, the baby's hepatitis B serologic panel will generally show some positive indicators.

It is crucial to emphasize that a positive hepatitis B serologic panel in a newborn does not necessarily mean the child is infected with the hepatitis B virus​ . Under normal circumstances, antibodies from the mother - such as surface antibody (anti-HBs), e antibody (anti-HBe), and core antibody (anti-HBc), including the e antigen (HBeAg) from mothers with hepatitis B - can pass through the placenta into the umbilical cord blood . For instance, an HBeAg-positive mother can transmit HBeAg to the newborn, but this positivity usually does not last beyond 3 months . Similarly, anti-HBs, anti-HBe, and anti-HBc can all cross the placental barrier, but anti-HBe positivity typically does not persist beyond 1 year of age, and anti-HBc positivity usually clears by 2 years of age.

The markers that most strongly indicate a true hepatitis B virus infection are the hepatitis B surface antigen (HBsAg) and HBV DNA. Generally, these do not pass through the placenta but can be transmitted through vertical blood transmission. Therefore, even with maternal blocking measures, a small number of newborns might test positive for low levels of HBV DNA and HBsAg. However, about 90% of these infants will see their HBsAg and HBV DNA clear up and turn negative by 7 months of age. Consequently, testing newborns for HBsAg and/or HBV DNA is not recommended. If venous blood tests still detect HBsAg and/or HBV DNA positivity between 7 to 12 months of age, then the child is considered to have a chronic hepatitis B virus infection.

The Wuwei Medical Science Institute Hepatobiliary Center is currently a National Hepatobiliary Disease Prevention and Treatment Technology Demonstration Base. Comprising the First Department of Liver Diseases, Second Department of Liver Diseases, Third Department of Liver Diseases, Fourth Department of Liver Diseases, and the Second Department of Hepatobiliary Surgery, it comprehensively conducts clinical and scientific research work in the field of hepatobiliary diseases, including liver protection, antiviral therapy, artificial liver support systems, surgical procedures, interventional therapies, minimally invasive techniques, iodine seed implantation, targeted and immune therapies, cellular immunotherapy, and stem cell transplantation. It is one of the few single-disease centers in China capable of perfectly achieving integrated diagnosis, treatment, and whole-process management for hepatobiliary diseases. The center undertakes multiple real-world research projects and several national public welfare projects.
