Medical Staff and Patients Join Hands to Defeat Cancer - Fight Against Pancreatic Cancer

This is a real case that occurred in the Radiotherapy Department I of our hospital, telling the story of an out-of-town patient who came to our hospital for medical treatment out of admiration for the hospital's expertise. The patient was clinically diagnosed with advanced pancreatic cancer. The family members had consulted treatment plans and prognoses in many large hospitals in big cities. They were well aware that pancreatic cancer is known as the "king of cancers", and the prognosis for advanced cases is extremely poor, with a general survival period of 3 - 6 months. However, the patient and family members did not give up any treatment opportunity and had firm confidence in defeating the cancer.In addition, the patient and family members highly recognized and trusted the medical staff. The doctor-patient relationship was harmonious, and they were grateful to each other. This also enabled the comprehensive treatment plan formulated through multidisciplinary consultation to be fully implemented and carried out. We adopted a comprehensive treatment model of "carbon ion radiotherapy combined with chemotherapy and targeted therapy". The patient achieved very good treatment results. Both the patient's family members and medical staff believed that a miracle had occurred.

[General Condition and Clinical Diagnosis]

General Condition: Patient Zhang XX, female, 62 years old, has had intermittent upper abdominal distension and pain for more than half a year, and was diagnosed with pancreatic cancer 6 months ago.

Clinical Diagnosis:

  1. Malignant tumor of the pancreatic head with extensive liver metastasis, cT4N2M1, stage IV, KPS score: 80 points
  2. Incomplete obstruction of the common bile duct

[Treatment Plan]

  1. Place a stent in the incompletely obstructed common bile duct to relieve obstructive jaundice.
  2. Perform local heavy ion radiotherapy on the primary lesion of pancreatic head cancer. The prescribed dose is 36 Gy (RBE)/12 fx, 3.0 Gy (RBE)/fx, followed by a reduced-field boost of 22.5 Gy (RBE)/5 f, 4.5 Gy (RBE)/f. This is to prevent tumor progression from aggravating the local compression of the common bile duct and the blood vessels at the porta hepatis, and to ensure unobstructed hepatic blood circulation and bile excretion.
  3. The systemic treatment plan is as follows: NASOX regimen (Oxaliplatin 100 mg on day 1 + Irinotecan liposome 86 mg on day 1 + Tegafur gimeracil oteracil potassium capsules 60 mg twice daily on days 1 - 7, every 2 weeks) + Nimotuzumab 400 mg by intravenous drip, once a week.
